Green Arrow - hard copy entry form needed

Does anyone have a hard copy entry form or can point me to one online to download?

Cant see anything on BC website

For the Green arrow event in Hertford there's no U8 Grass track option online and the organiser has said she doesn't really want U8's on track, but Logan can race as an U10 if I send a hard copy entry form and a cheque by Monday!
Not making it easy are they :)

    Update - Well that's odd. I eventually found something I could use and filled it all in except the 'enclosed fee' bit and went back on BC website to check the fee. I tried just for the hell of it to enter him and 'hey presto' its let me enter him. Very odd. Confirmation of entry and payment so all is good :)
